SERVICES: Community Choices provides one-on-one telephone/in-person counseling and library materials on a broad range of topics of interest (such as Medicare, Medigap insurance, Social Security, social services programs) to older adults and their caregivers, and finds callers help and matching services through programs such as
- CHOICES ("Connecticut's program for Health insurance assistance, Outreach Information and referral Counseling Eligibility Screening")
- Medi$ave, which educates consumers about reading their health care statements to prevent errors in the Medicare and Medicaid systems,
- Home Share Match, which helps match those in search of affordable housing with home providers in need of extra income or services;
- Seniors Guide to Services -- Compilation of resources and contacts, available in print and online
Home Care Program for the Elderly and Respite Care Management Programs provide assessment of need, ongoing care management and subsidized home care services to eligible individuals age 65 and older, as well as supporting their caregivers. NOTE: As of May 2009 the Alzheimer's Respite Program is closed to new clients due to State budget cuts.
